This template is used when a family, acquaintance, or media contact asks to submit a set of 10 yes/no questions
for PK‑based verification (predefined yes/no responses). The goal is method‑based documentation:
clear criteria, testable data points, and transparent logging.
Scope & Boundaries
- This is a verification request format (yes/no). It is not therapy, prediction, or advice.
- Please avoid medical, legal, financial, or safety‑critical questions.
- Questions must be specific, non‑guessable, and verifiable by you (the submitter).
- Responses will be logged in a time‑stamped Evidence Table. Answer keys are checked after logging.
A) What to Send (10 Questions + Answer Key)
- 10 yes/no questions (numbered 1–10), written clearly and unambiguously.
- A separate Answer Key indicating the expected answer for each question (YES/NO).
- For each question, include a short note on how you can verify the answer (e.g., document, witness, family record).
- For each question, add a Type label:
- Private‑Verifiable (known to family/close acquaintance, verifiable by you)
- Public Anchor (publicly verifiable baseline fact)
- Control / False‑Premise (expected NO; used to reduce chance hits)
B) Question Design Rules (to strengthen proof‑grade quality)
- Be specific: one idea per question; avoid compound questions.
- Avoid leading phrasing: do not suggest the desired answer.
- Avoid emotions-only questions: questions must be verifiable, not purely subjective.
- Avoid predictions: no future events, timelines, or speculative outcomes.
- Mix types: recommended mix is 6 Private‑Verifiable + 2 Public Anchors + 2 Controls.
- Randomize order: do not group similar questions together.
C) How We Run the Session (logging & integrity)
- Predefined code: The yes/no code is agreed in advance (e.g., 1 thud = YES, 2 thuds = NO).
- Blind checking: The Answer Key is not consulted during the session.
- Evidence Table logging: Each question is logged with timestamp, exact wording, and response.
- Repeat rule: If a response is unclear, the question may be repeated up to two times maximum.
- Stop rule: If conditions are disturbed (noise, interruptions), the session is paused or rescheduled.
